Regence BlueShield paid $2.17 billion for member health care in Washington in 2025

SEATTLE – Regence BlueShield paid $2.17 billion in health care for its fully insured members in 2025, or $4,834 per member (excluding the impact of reinsurance), according to unaudited figures filed today with the Washington Office of the Insurance Commissioner.

Health care costs per member with comprehensive coverage increased over 15% from 2024 and continue accelerating at rates higher than historical norms. The rise in claims costs continues to be driven by significant increases in   cost of care and prescription drugs. Regence grew its membership, ending 2025 serving 1.58 million people, including self-funded and BlueCard® members.

On total revenue of $2.38 billion, Regence had an operating loss of nearly 8% and despite investment income, a net loss of 3.1%. Although 2025 saw a slight increase in membership and positive investment returns, the loss is largely due to increasing health care costs. The company’s capital reserves remain appropriate, ensuring stable health care coverage and the ability to pay member claims. As a tax-paying, not-for-profit health plan, this critical safety net is Regence’s only source of funds for investing in new solutions to address health care challenges, meet the changing needs of consumers and pay higher than expected medical costs.

Regence incurred approximately $33.9 million in federal and state income taxes and fees in 2025.
